Electronic Statements Disclosure

E-Statement Disclosure Agreement

By accepting the terms and conditions of this agreement, you are electing to receive your deposit account statements and certain disclosures electronically in lieu of paper copies.

Scope of Consent

Your consent to electronic delivery covers the following categories of communications:

• Monthly or quarterly statements in connection with your deposit accounts
• Images of checks paid against your account during the statement period
• Disclosures that are provided to you on your statements
• Notices, disclosures, or account change in terms information (if we so choose)

If your account is a joint account with another person(s), consent to electronic delivery of your statement by one person shall apply to both or all of you. E-Statements sent to one joint account owner shall be considered communicated to both or all owners.

You understand that once we process your E-Statement request, we will discontinue mailing printed account statements and check images after the first month.

It is your sole responsibility to protect your sign-on information from unauthorized persons. It is your responsibility to ensure that the electronic statements cannot be intercepted or viewed by others. The Community Bank of Elmhurst has no control as to the persons who have access to your personal computer and sign-on information and will not be liable for any unauthorized access to your computer or sign on information. You understand that you have no expectation of privacy if the statements are transmitted over the World Wide Web. You further agree to release Community Bank of Elmhurst from any liability if the information is intercepted or viewed by unauthorized parties. 

Statements will remain available on the system for 24 months

Notification of E-Statements

You must be enrolled in Community Bank of Elmhurst Online Banking, and must provide us with a valid email address for notification purposes in order to access your E-Statements. Each statement period you will receive an email notice advising you of the availability of your E-Statement. Upon receipt of that notification you may access your current E-Statement using Community Bank of Elmhurst online banking.

Right to Withdraw Consent

You may withdraw your consent to have your statements provided electronically at any time without penalty by selecting the “paper” delivery method for your account through Community Bank of Elmhurst Online Banking. Once your consent has been revoked, all subsequent statements and disclosures will be delivered to you in a paper format.

Requesting a Paper Copy of your statement

You may print your E-Statement for your records at any time. You may also request a paper copy of a statement by contacting Community Bank of Elmhurst (There may be a fee associated with this). Updating Your Contact Information

It is your responsibility to provide us with a complete and accurate email address, as well as a valid phone number and mailing address. You must notify us promptly of any change in your contact information. Your email address can be changed online via the Community Bank of Elmhurst Online Banking website.

Our Right to Terminate

We reserve the right to discontinue providing E-Statements, and revert to paper statements at any time. Should we decide to discontinue E-Statements you will be notified as is required by law.

Change in Terms

We will notify you of any changes in these terms of agreement, either by email or by U.S. mail, addressed to the email or physical address we have on record. If you do not agree with any changes to terms, you may withdraw your consent to receive E-Statements in the manner that is described above.

These requirements may change periodically. We will notify you if these requirements change.

Compliance with Applicable Laws and Regulations

All electronic statements shall be in full compliance with applicable laws and regulation. The provisions in this agreement are part of and in supplement to Community Bank of Elmhurst terms and conditions for depository accounts and are subject to all the same provisions. Each account that you designate to be included within the E-Statement service is also governed by the terms and conditions otherwise applicable to the kind of account as separately disclosed to you, either in the Account Agreement and Disclosure, Schedule of Services and Fees, application and enrollment forms, our Privacy Policy, or any other written disclosures.

Verification of Ability to Obtain Statements Electronically

By agreeing to the terms and conditions of this disclosure, you are agreeing to accept email messages sent to the email address provided. You are requesting electronic delivery of the communications listed above. You understand that Community Bank of Elmhurst will discontinue mailing statements to the address on record. You are acknowledging that you have access to a computer that meets the technical requirements listed above, including access to a printer or the ability to download information to keep copies for your records.
